HEALTH SERVICES
The Health Services team promotes a resident’s lifelong physical and emotional well being by providing accessible, caring, comprehensive, and cost-effective primary health care.
Services Include:
- Coordinating all health-related appointments
- Promoting healthy behaviors and lifestyle choices
- Handling Villager health complaints
- Basic first aid
- Medication oversight
- Short-term minor illness treatment
- Hospitalization liaison and return to campus coordination
- Villager and staff training on health-related topics
SOCIAL SERVICES
The social services team advocates for each individual and fosters a trusting environment. Case Managers strive to promote self-sufficiency and self-empowerment.
Services Include:
- Communicating with families
- Individual support sessions for Villagers, as needed
- Support for setting reachable personal goals
- Budgeting support and processing money requests
- Travel planning and family visits
- Case file management
- Individualized Service Plans (ISPs)
- Helping with Medicaid/Medicare and Social Security forms
